Abstract

This project provides a framework that can acts as a guideline as there are numerous RCA tools that have been introduced and developed which might be a burden and troublesome for some engineers to choose from to suit their application in industry. To develop the framework, this project was carried out in three main steps. In the first step, a comparative study with selected criteria is done through literature review and root cause analysis performed with Fault Tree, Tripod Beta and Apollo technique. In the second step, a framework is developed after the comparative study has been done where each of RCA tools’ strength and limitations are known. Finally, the framework is converted into a simple graphical user interface to simpler the selection process of right RCA tool for their application. The result of this project shows that each of the selected RCA tools has their own strength and weakness where their application depends on the nature of the incident.
